Even though there can be arguments on the ranking of these reasons, I believe these reasons can be of great help in understanding what and where it went wrong with the Project.

• Lack of Planning
• Poor User input (mismatch of expectations)
• Lack of senior management/executive support
• Poor definition of project scope
• Unrealistic timescales
• Lack of, or inadequate, resources
• Incomplete or changing requirements and specification
• Lack of leadership and/or communication skills
• Lack of project specific skills/competence
